The Well Report Submittal Process - You will be asked to:

Answer Two Questions

1.	Is there an online form for the well type of your Notice of Intent (Notice)?
2.	How do you group your wells (if more than one) so you can report them to Ecology?

Complete Three Steps

3.	Complete an online form for your well or for each group of “identical” wells. (About five minutes per form)
4.	Submit your form and receive both a construction and decommissioning well report for all the wells included in the report.
5.	Repeat steps 3 and 4 for each additional group of constructed wells for that Notice.
