Notice of Intent Forms

Prior to filing a Notice of Intent (NOI) with us, check with your local permitting agency to ensure you are drilling in compliance with the requirements. Some areas may require additional fees or a property owner to connect to a public water supplier.

You'll need the following information:

  • Tax Parcel Number
  • Property Owner's Mailing Address
  • Well Construction Site Street Address
  • Proposed Type of Well (i.e. Domestic, Group Domestic, Stock Water, Irrigation, etc.)
  • Public Land Survey Information (Township, Range, Section, Quarter and Quarter-quarter) – This information can often be found on the county assessor’s site through either the legal description or their mapping tools. Wells Without Fees

Instructions
  1. Fill out the form using the appropriate link below.
  2. Provide a valid e-mail address for us to send your validation.
  3. As soon as Ecology receives your form, we will send your validation to the email address you provided.
  4. Environmental Investigation borings and Geotechnical Soil borings are decommissioned immediately after construction; we recently changed the way our forms page works to take this into account. You will now get two PDFs when you complete your Notice of Intent - one for the construction and one for the decommissioning. You no longer need to fill out two forms with the same information, just fill out your Notice of Intent to construct and we'll take care of the rest.

That's it! No need to mail anything and you won't have to wait for us to mail back your validation. Remember this is ONLY for wells that do NOT require a FEE. Using electronic forms for wells without a fee will save at least 7 to 10 days of process and mail time.

Wells Being Decommissioned

Now all decommissioning forms are on one single page. Whether you have the original Notice of Intent number or not, use this one online form to tell us about the well you are decommissioning.


Wells With Fees

Instructions
  1. Fill out the form using the appropriate link below.
  2. Provide a valid e-mail address for us to send your validation.
  3. Pay by Mail or Online:
    By Mail Print and mail the form along with the appropriate fee.
    Pay Online The Department of Ecology is now accepting online payments. You can use a Credit Card or your Bank Account.
  4. As soon as Ecology receives your form and processes your payment, we will send your validation to the email address you provided.

You won't have to wait for the validation to come in the mail! Using electronic forms for wells with a fee will save at least 3 to 4 days of process and mail time.
